Five Whys Canvas

This canvas separates root causes from symptoms. It provided an added safeguard to problem definition -ensuring we are addressing the real problem.

Leading Practices:

Use the canvas to interrogate the problem: Each “why” should make the problem harder to ignore and more precise, not more comfortable or abstract.

Force evidence at every step: Use concrete examples, field observations, or data at each “why.” Unsupported answers signal assumption, not insight.

Allow multiple causal threads to coexist: When causes diverge, branch the analysis. Collapsing complexity into a single cause can indicate oversimplification.
